Affordable Care Without Dental Insurance
Focusing on skill and gentle care provided in a warm, relaxing atmosphere, Cornerstone Dental uses advanced techniques and technologies to provide the ultimate in dental care. Our goal is to provide the best cosmetic, restorative and family dentistry services available.
For patients without dental insurance we offer the Cornerstone Club. As a member of the Cornerstone Club, patients receive optimal dental care at a significant discount by eliminating the high costs and hassles associated with billing insurance companies. We offer a Standard Plan, benefiting adults and children with standard dental care needs as well as a Periodontal Plan for individuals with a diagnosis of gum disease who require more frequent visits.
Cornerstone Club Membership
Good oral hygiene is important and we don’t want your teeth and gums to suffer because you don’t have dental insurance. That’s why we have designed the Cornerstone Club for our patients without dental insurance.
Cornerstone Club is an annual reduced fee dental plan that allows individuals and families to receive optimum dental services from Cornerstone Dental at a discounted price. This is not an insurance plan and cannot be combined with any dental plan.
Standard Plan
Our standard plan includes two routine cleanings, up to two regular exams, all recommended diagnostic X-rays, two fluoride treatments, and one emergency exam (if needed). Members will also receive a 10% discount on all restorative services.
A value of $750 or more, and you receive our Cornerstone Club discounted prices.
Per Adult Membership: $490 by cash or check or $510 by credit or debit card
Per Child Membership: $422 by cash or check or $439 by credit or debit card
Periodontal Plan
Our periodontal plan includes four periodontal maintenance therapy treatments, up to two regular exams, all recommended diagnostic X-rays, two fluoride treatments, and one emergency exam (if needed). Members will also receive a 10% discount on all restorative services.
A value of $1,200 or more, and you receive our Cornerstone Club discounted prices.
Per Person Membership: $964 by cash or check or $999 by credit or debit card
Exclusions
Club Benefits are applicable only when services are provided by Cornerstone Dental. It is possible that a club member’s treatment requirements may be outside our realm of expertise or capabilities. In this event, the member may be referred to other dentists or specialists.
Club benefits and discounts cannot be used when treatment or services are provided by another dentist or specialist to whom we refer.
Club benefits are not to be used in conjunction with any other discount or promotion offered by Cornerstone Dental.
The 10% discount on restorative services applies only to those services and cannot be used toward the purchase of products.
Membership fees for the Cornerstone Club are subject to change.
Stated fees are discounted fees based on payments made via cash or check and credit or debit card transactions only. Use of extended financing options such as CareCredit will affect discount percentages.
Club membership benefits are not to be combined with insurance coverage.
Club membership benefits start on the day membership is paid in full and are in in effect for 1 calendar year.
Club membership fees are non-refundable; even if benefits are not utilized.
Club plans DO NOT include full mouth debridement or scaling and root plaining procedures.
At our discretion, patients who fail to show for an appointment or give less than 24 business hours notice to reschedule will be charged a failed appointment fee or forfeit a prophylaxis (cleaning) or a periodontal maintenance.
